Osteochondropathy of the Ischiopubic Synchondrosis

This condition was first described by Van Neck as a rare localization of osteochondropathy, which has been confirmed by histological studies. The process can be unilateral or bilateral, affecting the junction of the ischial and pubic rami of the pelvis. Normally, synostosis of these bones in children occurs between the ages of six and ten, whereas the disease typically manifests between eight and twelve years of age (Reinberg S.A., 1964).

Children complain of pain in the Perineum, which sometimes radiates to the thigh. Visual inspection of the perineal area reveals no pathology. Only through Palpation and digital rectal examination can a rounded, painful mass be detected at the junction of the ischial and pubic rami.

An anteroposterior plain radiograph of this area reveals a fusiform thickening involving the bone. The Bone Structure in this region appears blurred, with mottled foci of necrosis and micro-sequestration. Such structural changes confirm osteochondropathy and rule out a bone tumor.

Treatment. The child is excused from physical education classes at school, and heat therapy is prescribed. The prognosis is consistently favorable, and patients recover without any adverse sequelae.
