Surgical methods
To terminate a Pregnancy in the late stages, a mini-Cesarean Section is used. This is a cesarean section performed before the 28th week of gestation when there are contraindications to other Methods of Medical abortion and when a rapid termination of pregnancy is required. This Procedure may also be indicated if there are grounds for concurrent tubal sterilization or surgical intervention for any genital neoplasm, alongside the need to terminate the pregnancy.
A mini-cesarean section can be performed either vaginally or via a transabdominal approach.
Vaginal mini-cesarean section is used when the gestational age is 16–20 weeks. Several techniques for vaginal cesarean section have been proposed, The Essence of which involves incising the cervix and the lower uterine segment to create access to the uterine cavity. The fetus and Placenta are then removed through the created opening. The final stage of the procedure is the restoration of the integrity of the uterine and vaginal Tissues.
Transabdominal mini-cesarean section is most frequently performed at 20–28 weeks of gestation. The surgical technique is nearly identical to that of a standard full-term cesarean section.
Despite the high efficacy of this method for late-term pregnancy termination, it must be borne in mind that hysterotomy is a rather traumatic procedure. Even with modern surgical techniques and advanced anesthetic care, complications are still possible during the surgery, in the immediate postoperative period, and in the long-term follow-up.
